PRIVATE HEALTH REBATES
Most Private Health companies provide rebates for Exercise Physiology.
If you are unsure whether or not your provider does get in contact today with us today, or alternatively contact your provider and see if you are covered under these codes: 102, 202, 302, 402, 502
We have HICAPS available so all claims can be processed on the spot.
ACCEPTED REFERRALS
We accept a range of referrals including and not limited to:
Chronic Disease Management (CDM plans):
Under the CDM plans any patient that has a chronic medical condition for over 6 months is eligible for sessions organised through their local GP.
Diabetes Chronic Disease Management plan:
Patients with Type 2 Diabetes are eligible under this plan for an initial session and 8 subsequent group sessions, organised through their local GP.
ACCEPTED REFERRAL TYPES

Work Cover: We are registered Work Cover providers and accept referrals from eligible clients

NDIS: We are registered NDIS providers and accept referrals from eligible clients

Veterans Affairs: Entitled DVA patients may be referred by their GP to an Exercise Physiologist for clinically necessary treatments through a D904 referral.
PRICING
Chronic Disease Management (CDM plans):
Initial CDM Consultation 1hour ($53.00 gap, total cost $105.00)
Subsequent CDM 30 min ($20 gap, total cost $72.95) 45min ($32 gap, total cost $84.95)
Diabetes programs (in conjunction with dietitian).
Private:
Initial Consultation (60 minutes) $105,
Subsequent Consultation (45 minutes) $85.
Subsequent Consultation (30 minutes) $70,
Group classes $15 with packs available (10 classes for $150)
